Description
The aim of this service is to provide a primary community based services to the registered population of the practices who are not participating in NHS Nottingham City CCG's Primary Care Patient Offer (PCPO) and therefore by default are not commissioned to deliver these services to their own registered patient population. The services to be provided include:
• Phlebotomy - clinic based (for 12+ years) and domiciliary service encompassing all blood sampling for investigations and follow-up. The service excludes the taking of bloods for Warfarin anticoagulation therapy, bloods required as part of a shared care protocol and emergency bloods required on the same day / within 24 hours.
• Treatment Room services - to provide a minor injury and wound care treatment room service for patients requiring minor interventions including minor lacerations, partial thickness thermal burns and scalds, minor trauma to hand, limbs or feet, postoperative care for patient able to attend a clinic and a dressings service.
• Ear Irrigation - to patients aged 12+ years
• ECG - to patients aged 18+ years for routine screening of ambulant patients who are able to attend clinic. This service is not for emergency ECGs
• H Pylori - for patients who are under Patient Specific Directive (PSD) requiring this test.
• Prostate-specific antigen (PSA) monitoring - for patients with a confirmed diagnosis of prostate cancer who can be managed in the community, the service will provide PSA monitoring for investigations and follow-up.
The objectives are to:
• To provide a timely service to patients registered at one of these GP practices
• To provide a primary community based service, reducing the need for unnecessary hospital attendance or admissions
• Patients are encouraged and supported to self-manage their conditions in the community
