The Authority is seeking to appoint a capable and competent supplier for the provision of a Discovery phase, including development of prototypes and user research, exploring how Social Care / Domiciliary Care providers can receive notifications when their clients / customers have been admitted into acute NHS care, and have constant communications of their likely discharge date and time.
