Description
Bath and North East Somerset, Swindon and Wiltshire Integrated Care Board ("BSW ICB") are seeking to commission a provider to carry out Continuing Healthcare ('CHC') Assessments for approximately 150 individuals.
The assessments are a variety of those triggered by a positive CHC Checklist for all client groups, with a significant number being individuals with a diagnosed learning disability. There are also a number of individuals in receipt of CHC Fast Track funding who require the consideration of CHC eligibility to determine their ongoing funding stream. The assessments will include the completion of a Decision Support Tool to cover the period from the positive CHC Checklist to the current day.
The successful provider will comply with the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (2022 revised).
The successful provider will be expected to:
• Assess and record robust information and evidence the current level of need individuals.
• Complete the DST 2022 template
• Project completion within 6 months of the start date.
• Ensure effective engagement at all stages with key stakeholders.
• To book and coordinate assessments.
• Evidence and record recommendations to Caretrack.
• Record FNC outcomes on the BSW ICB FNC determination form.
• Conduct face to face CHC assessments for individuals residing in their own homes.
• Report KPI achievements weekly.

The contract will be for 6 months with an estimated budget of £101,000 exclusive of VAT.
This procurement is being carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioners.
