Description
NHS Nene Clinical Commissioning Group, working in partnership with Northamptonshire County Council and NHS Corby Clinical Commissioning Group, is seeking expressions of interest from support providers with experience of supporting individuals with mental health needs to provide Intermediate Support to working age adults with mental health needs covering Northampton, Daventry Towcester and South Northants. The contract will be for an initial 1 year period. The financial envelope will be in the region of £350K. The support provider will have experience in working with individuals in their own homes and in the community supporting people with advice, information and support to maintain their tenancy, budgeting, benefit advice, advice and encouragement in accessing community facilities, and medical appointments, monitoring mental health and working with local Health and Social Care Staff to share any concerns as part of an agreed support plan.
The main aim of the Intermediate Support service is to offer time-limited support to enable individuals who have mental health needs to meet identified outcomes. This service will be available up to a maximum of 6 months aiming to improve health and wellbeing, enable greater independence, resilience and social inclusion and help to divert from requiring secondary mental health services.
The specific objectives of the Intermediate Support service are:
• To prevent hospital admission and to enable discharge back to the community following a hospital admission
• To promote and support with healthy lifestyle choices
• To maximise personal and household income, to support with reducing household and personal debt
• To support with accessing local community opportunities and services (universal services) leading to a greater independence and social inclusion
• To advise and support with accessing educational opportunities
• To advise and support with accessing opportunities for employment

If a provider cannot cover the whole geographic area with their specific service, there will be an expectation that joint working will occur with other organisations offering the same, or similar services, to cover this potential gap.
Potential providers with relevant capability and experience to deliver this service are required to express an interest in bidding for this service should the Commissioning Groups decide to carry out a competitive tender exercise and publishes a Contract Notice for this requirement.
