Description

Medway Council, working with NHS Kent and Medway (NHSKM) wish to procure a new Wellbeing Navigation Service to commence on the 1 April 2023, to ensure continuity of service. The involves joined up working with system partners throughout Medway in order to improve outcomes for all Medway residents, to stay well and live independently, helping those individuals with conditions that could be preventable, those with long-term conditions and the ageing population to get care as quickly as possible, from the best resources. The service will support individuals to navigate the system, and access support to improve their health and wellbeing.
