Description

The service provides:
Consultation, advice and identification of potential referrals for triage assessment;
Client assessment and recommendations about appropriate care pathways; developing strategies for staff on how to respond to hostel residents with complex behaviours in order to improve client engagement and outcomes;
Liaison and attendance at case conference meetings;
Structured training, advice, support and supervision to staff teams.
This notice is to confirm the extension of the existing contract for a further period of 10 months to allow for discussions on future service design and any future procurement process to be completed.
