Description

East Sussex County Council intends to commission a service provider to design and deliver an integrated lifestyle service. The service will support people in East Sussex to make changes to their lifestyle behaviours to improve health outcomes, and reduce their risk of developing conditions such as diabetes, cancer and heart disease. The Integrated Lifestyle service commissioned through this tender will form an integral part of the health and care system in East Sussex.

The service will provide:

o One-to-one core behaviour change support

o Specialist Stop Smoking support

o Tier 2 weight management support for adults

o Community NHS Health Checks

o An evidence-based training programme which supports the development of community and personal resilience. The development and delivery of this training programme will be centred around Making Every Contact Count (MECC) approaches and support to utilise asset based approaches which recognise the strengths of communities, networks and individuals in health, care practice and wider public health workforce practice.

o System leadership and support to other providers of lifestyle change services, and

o The development and delivery of Public Health campaigns and associated activity to support lifestyle change; develop awareness of the impact of lifestyle on health and increase service uptake.
