Description

Leeds CCG and Leeds City Council are wishing to engage with providers interesting in establishing a broad partnership to design and develop a new model of Intermediate Care for the city for 2023 onwards.

An engagement event will be held via MS Team on Thursday 4th November 2021 (11:00 to 12:30) to outline the approach to be taken. This will be followed by a series of workshops to co-design an integrated and collaborative model. If you would like to attend the engagement event, please register your interest by sending an email to leedsccg.procurement@nhs.net. with the email addresses of the people wishing to attend.

Once registered, you will be sent an MS Teams invitation.

Please note that this notice is solely for the market engagement. An expression of interest in this engagement exercise will not automatically entitle involvement in any future procurement exercise. Any subsequent procurement (should one take place) will be advertised separately and

The objectives of the Leeds Intermediate Care Remodelling programme are to:
- Implement a single integrated model of intermediate care that embraces bed-based, homebased ntermediate care and re-ablement
- Shift the focus of intermediate care from a bed-based response to a care at home response
- Regain increased focus on admission avoidance as well as facilitating hospital transfers of care
- Reduce the intermediate care length of service through focussed intensive therapy approaches
- Simplify the pathway into and through the new integrated service model to provide the best possible experience for people and for professionals
- Simplify the pathway out of the new integrated service to ensure that people have the right ongoing care, support and connection to their communities
- Achieve the best outcomes for people within available resources.
It is intended to establish a partnership of interested stakeholders to commence the co-design of a new model through a Programme Board and events for stakeholders.
