Description

Hertfordshire County Council (the \"Council\") covers a population of 1.2 million. Hertfordshire is a two-tier authority with ten district and borough councils providing complementary local services to residents. There are two NHS Clinical Commissioning Groups covering the county.

The Council provides a range of services, including social care for adults and children, schools and education, public libraries, public health, highway and road maintenance, environmental services, trading standards and household waste sites as well as a fire and rescue service.

In Nov 2014 the Council awarded a new contract to one care provider to deliver stroke specific enabling support at home. The contract was split in to two parts, East and North Herts CCG area and Herts Valleys CCG area.

The expectation is that Provider staff will co-locate and work closely with and under the direction of the multi-disciplinary team (MDT) which includes OT's, physios, community nurses and social care managers. The MDT has two bases, one in East & North Hertfordshire and the other in Herts Valleys.

In line with the requirements of The Care Act 2014, Care Providers are required to focus on the well-being of the individual and the outcomes they want to achieve, helping them to have choice and control over their support and care needs. Additionally the multi-agency strategy Ageing Well in Hertfordshire 2014-2019 sets out key priorities to keep older people well, both physically and emotionally, enabling them to live in their own home for as long as they wish.
