Description

ASC089 - Promoting Independence - Care at Home Service

The Metropolitan Borough of Sandwell wishes to appoint 6 Contractor(s), which are registered with the Care Quality Commission, to provide a Promoting Independence - Care at Home Service in Sandwell.
The service will be provided on a short term basis to people who require care in their own home to ensure that their independence is maximised and that those people who formally enter into the care and support system do not unnecessarily grow dependent on it but strive to maximise their strengths and achieve their outcomes.
The Service will aim to reduce or delay unnecessary admission to acute services.
Tenderers should note that the Council has set rates for each element of the service - please refer to the ITT documents for further information in relation to costings.

The duration of the contract will be two (2) years from 3 February 2021 to 2 February 2023 with an option to extend by up to one (1) further, 3 February 2021 to 2 February 2024 (unless it is otherwise terminated in accordance with its terms or it is otherwise lawfully terminated).
The Council intends to use an evaluation model based solely on quality. The criteria, on which the tender will be evaluated, inclusive of weighting attached to each, are included in the Invitation to Tender (ITT) documentation.
