Description

NHS Dartford Gravesham and Swanley Clinical Commissioning Group (DGS) are seeking to commission a Diabetes Service with the aim of improving the health outcomes for those patients with diabetes whose care needs are beyond routine management in primary care but do not require treatment in an acute setting. The Tier 3 service will have a system leadership role in supporting and developing the capabilities of General Practice, driving integration across organisations and facilitating navigation of the diabetes care system, as well as in the direct delivery of healthcare services.
