Under the co-commissioning arrangements in place from April 2015, Clinical Commissioning Groups (CCGs) have delegated responsibility and together with NHS England wish to procure the following practices in the North Lincolnshire area.
NHS North Lincolnshire CCG is proposing to commission an APMS contract which is currently delivered from the Ironstone Centre, West Street, Scunthorpe, DN15 6HX. It will be a requirement that the services are offered from the current premises. As at 1 October 2015, the list size of the practice is approximately 5,290 patients.
The aim of the service is to deliver essential, additional and enhanced services to the registered patients of the practice.
It is proposed that the terms and conditions of service, including tariff arrangements, will reflect those contained in General Medical Services (GMS) contracts.
The commencement date for the practice will be 1 May 2016.
