Description

The Domiciliary Care Rapid Discharge Service is a Covid-19 response short-term service put in place to support the timely discharge of service users who may have an eligible Care Act need or need support with discharge from a Hospital setting to their own home. The service will be used where a long-term package of care is likely to be needed. All service users discharged into this service will be followed up by a Hampshire County Council member of staff.

Individuals discharged into this service may be Covid-19 positive and there is an expectation that the service provider will accept these individuals into this service. The Service Provider is expected to manage this need appropriately and in line with NHSE guidelines.
