Description
NHS Shared Business Services, on behalf of NHS Leeds South and East Clinical Commissioning Group, are inviting suitably qualified and experienced Providers to submit framework applications in order to enable the establishment of a Framework for the Provision of Domiciliary Care Services (the Services).
The Framework for the Provision of Domiciliary Care Services will be split into two lots as follows:
• Lot 1 - Domiciliary Care for Continuing Healthcare Patients; and
• Lot 2 - Complex Continuing Healthcare Packages at Home Service.
The Services will support adults (aged 18 and over) assessed as eligible for Continuing Healthcare (CHC) and registered with a Leeds GP, or resident within the boundaries of the 3 Leeds Clinical Commissioning Groups.
The aims and objectives of the Services for both Lot 1 and Lot 2 are to:
• Ensure the Service User's needs, as identified by the Commissioner within the Care Plan, are met
• Ensure that care is delivered as identified within the Provider's Care Plan, through a range of interventions tailored to meet individual needs
• Enhance the quality of life of the Service User
• Support Service Users to live as healthy and independently as possible irrespective of their condition
• Ensure Service Users and carers experience person centred outcome focused care in which the Service User (and/or their designated representative) are involved and satisfied with the plan of care
• Provide appropriate levels of skilled and competent care staff trained in a range of care interventions to meet the wide range of physical, mental, emotional, cognitive and cultural care needs of the Service User
• In collaboration with health care professionals and/or other organisations, develop a Care Plan that meets the Service User's health and social care needs and evaluated at least monthly or more frequently as required to meet patient needs
• Ensure risks are identified and managed appropriately, in order to protect the safety of the Service User and care workers
• Contribute to a multi-disciplinary approach, ensuring effective communication with all Health and Social Care professionals involved in the care of the Service User
• Provide person centred information and support for Service Users, families and carers
• Ensure ongoing communication with all partner agencies or other services involved in the patient's care
• Facilitate rapid/early discharge from hospital and prevent inappropriate hospital admissions
The Frameworks which successful Applicants will be appointed to at the end of this Procurement will be for a duration of up to 3 years, which is intended to run from 1st April 2018 until 31st March 2021 with the possibility of one extension of up to 2 years, subject to any restrictions placed nationally by NHS England or the Department of Health and at the discretion of the Contracting Authority.

The Services to which this Procurement relates fall within Schedule 3 of the Public Contracts Regulations 2015 (the "Regulations").
