Description
We are looking to award a single supplier to deliver remote monitoring hardware and software to support its programmes across West Yorkshire.
Leeds
Leeds already offers a virtual ward hospital @home model for frailty and respiratory and has plans to expand the bed base and introduce mental health input to enhance the frailty pathway. These services deliver both step-up and step-down pathways but are not currently enabled with the use of remote monitoring technology.
Leeds plan to expand their virtual ward pathways, and further increase capacity through the implementation of a new remote monitoring virtual ward hub that will primarily provide step-down pathways to support patients across the following specialities:
• Frailty
• Respiratory
• Surgery
• Cardiac heart failure
Calderdale, Kirklees and Wakefield (CKW)
CKW Virtual Ward pathways include:
• Frailty
• Respiratory
Across the Calderdale, Kirklees and Wakefield (CKW) footprints, there is an existing virtual ward for frailty, providing safe and optimal care to frail older adults who are presenting with urgent or emergent care needs. This service is for adult patients aged over 75, and there are plans to expand the bed base, in addition to offering this service to patients from the age of 65. CKW also plans to open respiratory beds for patients aged 16+ with chronic respiratory disease/exasperation and COPD.
There is potential for additional care pathways to be included in the future such as cardiology, end of life care and diabetes management.
Bradford District and Craven
Within the programmes, there could be scope to extend the supplier contract to include Bradford District and Craven virtual ward providers within the contract period. Bradford District and Craven who already have virtual wards operating for frailty and multi-specialty pathways will be expanding their bed base and increasing their capacity within their multi-specialty pathways covering end of life and palliative care, general surgery, cardiology, diabetes, nephrology and orthopaedics.
