Description

East Berkshire CCG is seeking expressions of interest in a potential procurement for a service providing respite, support and advice to carers of those younger people diagnosed with dementia. This would be done through the provision of meaningful and age-appropriate workshops and/or 1:1 support as well as micro groups (2 people or more) for those that may have interests outside those offered in the workshops (i.e. golf). The service would also support the younger person with dementia transition into other supportive services when appropriate with the support and links into an admiral nurse. The service would be open to patients registered with an East Berkshire GP aged from 18 to 65 with a diagnosis of dementia.

The service would potentially start in April 2020 for a period of 1 year with the option to extend for one further year past this if KPI's are met.

The success of the service will be measured by by a variety of methods including (but not limited to) carers' feedback, the number and type of workshops established and the 1:1 and family support offer provided. The service would also need to provide links into an admiral nurse with other measures in line with the service above needing to be confirmed. The contract value will be approximately £120,000 with an estimated current case load of 40 patients.
