Description

Offer the most cost-efficient solution to meet the Council and CCG's legal duties regarding the provision of minor adaptations which meet the eligible care and support needs of B&NES residents of all ages.

The Service contributes towards Health and Social Care professionals' work in preventing admission to hospital, enabling early discharge from hospital, reducing the need for packages of care and support once home, and enabling people to maintain their independence and quality of life by providing appropriate equipment in a timely manner.
