Durham County Council is seeking a single provider to deliver both Lots as detailed below:

Lot 1 - County Durham Brokerage Service (Hospital to Residential/Nursing Care)
Lot 2 - County Durham Trusted Assessor Service

The Council is seeking a single provider to deliver these two separate services as they have close linkages. However, the Council may choose not to extend one of both Lots separately.
