Description

2.1. Bradford Council wish to procure a keyworker service for vulnerable families living in Bradford & District that meet the national 'Troubled Families' criteria and the locally set criteria. The Provider will provide key work and targeted youth support through a 'Think Family Model; with one worker, one family, one plan and actively support external Lead Practitioners in a Team Around the Family (TAF) approach.
