Description

Norfolk County Council has a requirement for community support services for people with mental health needs in Norfolk. The procurement is in 4 lots as follows:-

1. Lot 1 - West Norfolk

2. Lot 2 - Central Norfolk

3. Lot 3 - East Norfolk

4. Norwich locality intensive outreach

The service will:-

o Support people to live independently and safely in their own homes through the provision of support in their homes, including in specified supported living schemes, and the community.

o Enable people to recover and develop the capacity to live more independently in their communities.

o Develop the capacity of people to maintain their recovery and their ability to live as independently as possible.

In particular the service will:

o Create a pathway for people to move from residential care to supported living and then to independence in the community, where the support at each stage is provided by a team of staff whom the service user is familiar with

o Provide an integrated staff team where support workers are recovery focussed, are operating on the principles of psychologically informed environments and support people to develop their ability to do things for themselves

o Provide flexible levels of support to minimise the risk of crisis, to be able to provide more intensive support when needed and increase the potential for long term recovery.

o Where approaches have failed in the past, take a creative approach based on service users strengths and wishes to develop hope and resilience

o Know what is in the local community and be able to link people in with activities and friendship groups which offer sustainable support and use community assets. This will include locality based services for vulnerable women

o Utilise supported living schemes in the service as a 'hub' for support to service users in the community outreach service at evenings and weekends

o Provide support to and engagement with people who have dual diagnosis or who are difficult to engage to prevent them going into crisis and becoming 'revolving door patients'.
