Description
NHS Havering Clinical Commissioning Group (CCG) is seeking to commission an organisation that will support the delivery of the Post Diagnosis Dementia Support service. The service will be delivered and based in the London Borough of Havering and managed locally. The aim of the procurement is to procure a quality service that provide consistent, highly responsive, personalised and emotional support to people in the London Borough of Havering with a diagnosis of dementia and their carer. It also aims to ensure dementia patients and their carers feel more informed and confident about services available to them. The successful provider will have a good understanding of dementia and how this impacts on the life of an individual and their family. They will additionally receive referrals from various partners, manage caseloads, provide advice, guidance, and support to patients and carers and/or signpost them to other service providers. This may be provided through telephone, face to face (including home visiting where required) and group (peer) support. The duration of the contract is 3 years with the possibility to extend for 24 months. The anticipated contract value is £205,000 per annum.

The successful provider is expected to commence mobilisation of the project in April 2019, with the actual project delivery commencing on 01 July 2019.
