Description
Leeds is one of the four areas nationally being supported by NHS England in a development programme on Population Health Management. It has been agreed that £25k from NHS England will be available for each of the four LCPs to design and test interventions to support people living with frailty. The allocation of this funding has been agreed by the West Yorkshire ICS.
The intervention is aimed at identifying patients living with moderate frailty within the Seacroft area. The inclusion criteria states that the patients must be registered at a Seacroft GP practice, coded as moderately frail and have both COPD and CHD and aged over 65.
