Description
Lead, coordinate and deliver a Partners in Care model in Cheshire West and Chester over the 18 month period.
The project will consist of social care and health providers representatives working to develop Partners in Care- a membership organisation. Regular communication with Commissioners and other key personnel from the Council is required.
The project lead will ensure that the Partners in Care (PIC) will have a sound infrastructure over the next 18 month period, with a strong membership base.
To ensure that, at the end of the 18 month period of the project, the PIC will be tested and reviewed to ensure it is in a strong position to offer advice, support; deliver workforce development and training; actively participate in market shaping and market development and generate income from other sources.
The service will facilitate the development of overall market capacity through building the asset base of the sector. A key focus must be on enhancing the quality of the services for the benefit of people using them.
