Description
Leicestershire County Council wishes to seek suitable providers to deliver a pilot scheme for six short term residential care beds for individuals who require a short period of assessment to enable them to return home or to be moved into the correct setting for their needs. Typically, this scheme will be required following a hospital stay or where the individual has experienced a health or social care crisis in their own home. The focus of the unit is on social care interventions and approaches, rather than health care ones; the assessment will focus on an individual's social and environmental situation, helping to resolve social problems and achieve a maximum level of choice and control. A dedicated case manager will be responsible for the case during the admission. This procurement will be using the Light Touch Regime tender.
