Description

This tender is for a Short Term Reablement support service, provided in Care Quality Commissioned (CQC) registered residential care homes (and nursing provision if suitable). The service will support the discharge of individuals from hospital, for a temporary period of up to 6 weeks, with the aim to re-able the individual to be as independent as they were prior to admission to hospital and return to their home.
The overall aim of the service is to avoid unnecessary delayed hospital discharges, avoid unnecessary hospital admissions and ensure there is an efficient flow into appropriate accommodation of those with social care requirements.

The service will allow all key partners (hospital, commissioning, provider, primary care, occupational therapists and social workers) to build strong relationships between stakeholders which helps support safe and timely discharges over 7 days per week via a trusted
