Home to Settle Service (Reference 19.513)
NHS Brighton and Hove Clinical Commissioning Group
Request for Information (RFI)
The Commissioner is seeking the views of Healthcare Providers for a Home to Settle Service. The anticipated value of the Service is £300k-£400k a year. The Commissioner is considering a contract period of 3 years plus an option (option of the Commissioner) to extend the contract by up to a further 24 months. The requirement is for a Home to Settle Service for patients attending, or being discharged from Royal Sussex County Hospital, Brighton.
The Aims of the Home to Settle Service are:
To support patients who live alone or with a carer, to return home safely from hospital following A&E attendance or ward discharge.
To work proactively with discharge teams and other teams such as Accident and Emergency to support timely discharge from, or avoid admission to RSCH.
To carry out an initial risk assessment upon return home and agree any additional referrals/signposting as appropriate to ensure patients remain independent and rebuild their confidence.
To work collaboratively with key stakeholders to ensure patients are referred into appropriate services.
Service outcomes:
• Patients are provided with reassuring, safe and companionable car transport home from hospital;
• Patients are supported to return to their home safely with their immediate needs met;
• Patients receive the information, advice and support they need in order to prevent unplanned readmissions;
• Patients maintain/regain their independence and rebuild confidence;
• Patients are signposted to/integrated into services in the wider community;
• Reduce discharge delays;
• Reduce admissions for patients whose needs are more social than medical.
This RFI process is designed to help the Commissioner form a view of the best way to commission the service and is not the beginning of a tender exercise. A further tender advertisement will be issued as/if required.
Your feedback at this point will not have a bearing on any future Tender submissions you may wish to offer at a later date should the Commissioner undertake a procurement for this service. You will not be disadvantaged if you choose not to respond to this RFI but it will be helpful to understand your views at this early stage, so you are encouraged to respond as fully as you can.
