Description
East Leicestershire and Rutland Clinical Commissioning Group (ELRCCG) acting as Coordinating Commissioner on behalf of NHS Leicester City CCG and NHS West Leicestershire CCG across LLR are inviting expressions of interest from suitably qualified providers to deliver the Assessment and review of Eligibility for NHS Continuing Healthcare and Funded Nursing Care for Adults and Continuing Care for Children and Young People across Leicestershire: 1. NHS Continuing Health Care (CHC) and NHS Funded Nursing Care (FNC) for Adults including Personal Health Budgets (PHB). 2. Continuing Care for Children and Young People including PHBs. 3. Health Case Management for Pathway 2; HART and Nursing Home community reablement. LLR CCGs are looking to procure a combined end to end and lifelong service. The estimated contract value is £11,981,054 GBP over the 5 years .
The key aims and objectives include meeting the requirements of the following: - the National Framework for NHS CHC and NHS FNC (revised 2012); - the National Framework for Children and Young People's Continuing Care (revised 2016); - National Guidance and regulations regarding the implementation of PHBs including the NHS Direct Payments Regulations (Amended 2013); - Children and Families Act 2014; - The SEND Code of Practice January 2015; - Act as the health case manager for the reablement service for those individuals; - Assessing patients who have been identified as likely to be eligible for CHC, or who are placed in a nursing home and are eligible for receipt of FNC; - Reviewing those patients in receipt of fully funded CHC, including fast track funding, joint funded care packages, or FNC; - Assessing children and young people who have been identified as likely to be eligible for continuing care including those in transition; - Increase the number of individuals who are able to have a PHB; - Case managing patients whilst on the HART or Pathway 3 Nursing Home reablement pathway. This does not commit the CCGs to carry out a procurement process. Current services will remain in place during this period in line with existing contracts and Service Level Agreements.
The contract will be the NHS England Standard Contract for a proposed period of 5 years, with an option to extend for a further 2 years subject to satisfactory performance.

LLR CCGs are looking to procure a combined end to end and lifelong Personalised Commissioning Service that determines eligibility for newly referred individuals or review of eligibility for Continuing Healthcare (CHC) or Continuing Care (CC); brokerage packages of care and assessment of the suitability of packages of care for eligible individuals where this is not undertaken by the Local Authority (LA) and, where necessary, identifying and brokering care for those individuals who are not CHC or CC eligible but whose needs are of a complex nature as to warrant a contribution from the NHS towards their care.
