Description
Medway Council Public Health is looking to procure an organisation that works in street-based or supported accommodation settings to recruit a full-time Peer Outreach Coordinator and a full-time Peer Outreach Worker to recruit and manage people with lived and living experience to form part of a multi-disciplinary team (MDT) tasked with providing enhanced support for people who use substances and are at risk of or experiencing rough sleeping in Medway.
The Peer Outreach Coordinator will recruit peer mentors who have lived and living experience and coordinate staff and volunteers who engage in assertive outreach and in-reach. The role will also include partnership work with agencies that contribute to the Medway Multiple Disadvantage Team (MMDT), and representation at planning and review meetings.
The Peer Outreach Worker will support and manage peer volunteers in community settings and ensure work focuses on the aim of engagement with substance use and accommodation services.
This project will be an integral part of a newly formed MMDT including substance use, social care and mental health support in community settings. In conjunction with partner projects across Medway, the MMDT aims to reduce drug and alcohol-consumption and deaths, improve pathways between services, increase the number of people with complex needs accessing harm reduction interventions and treatment services, and reduce homelessness and rough sleeping.
The Contract is anticipated to commence on 1st June 2023 and will continue for a period of 22 months (10 + 12 months), unless terminated in accordance with the Conditions of Contract
