The Primary Care Personalised Care Programme aims to support people in Ipswich and East Suffolk living with complex health needs. Patients within the Top 5% complex cohort will be referred from Primary Care (GPs, PCNs, High Intensity User Groups, MDTs) and offered non-medical opportunities for social support. Specifically, enhanced opportunities through provision of interventions based on Creative Arts, Green Therapies and Supported Self-Management within the community. Success will be improved patient health outcomes, reduced primary care and other system utilisation rates and improved patient satisfaction including self-reported health and wellbeing scores. We wish to commission non-medical interventions to provide health outcomes to support adult aged people with complex health needs. The service(s) are based on enhanced social prescribing models for highly complex patients referred from Primary Care and should be designed to support patients in the long-term. This work is a pilot to understand the impact this level of support could provide and will therefore be a short-term project of 9 months.
The CCG are looking to commission supporting interventions in the following disciplines for our most complex patients who are adults experiencing social exclusion or isolation and suffer with multi-morbidities.
Creative Art Therapies - Evidence shows that access to arts opportunities and participation in the arts can "dramatically improve health outcomes and wellbeing, counter inequalities and increase social engagement". Being creative improves emotional and psychological wellbeing, builds self-esteem, mitigates social isolation and loneliness and helps to promote more cohesive communities.
Supported Self-Management interventions based on therapeutic relationships - Evidence shows personalised care, seeing a person holistically and working with them closely on a long-term basis can encourage, support and empower people to manage their ongoing physical and mental health conditions themselves. Health coaching, peer support and self-management education are systematically put in place to help build knowledge, skills and confidence.
Green Therapies - Evidence shows that Green interventions improve wellbeing and physical activity, reduces stress, depression, blood pressure, increases self-esteem, mood and wellbeing. Wellbeing is important to health as it increases life expectancy, improves recovery from illness and is associated with positive health behaviours.
We are looking for specialist experienced partners to deliver the three supporting interventions above to provide alternative non-medical interventions for patients to promote health outcomes. This is an opportunity to provide 'proof of concept' evidence to support longer-term funding for social prescribing initiatives and bridge the gap between statutory service provision and our wider voluntary and community based social enterprises.
