Description

Portsmouth City Council and Portsmouth Clinical Commissioning Group (CCG) are in the process of preparing to retender the Support at Home Service. They are looking to gauge the number of providers for this type of service who would be interested in working with us.

Portsmouth City Council and Portsmouth CCG are looking to work with one provider to deliver the support at home service.

The overall aim of the Support at Home service is to provide a responsive service to individuals who are medically fit for discharge from the Queen Alexandra Hospital to their own homes in both Portsmouth and Hampshire. It will support patient flow by providing practical and emotional support to those patients identified as benefitting from the service leaving the hospital.

The closing date for responses to this PIN is the 26th of August at 8am.
