Description

A new approach is needed to achieve greater levels of integration to bring positive benefits in terms of improving people's health, wellbeing and experience of care, particularly in wrapping services around people's needs and shifting the focus to keeping people well and happy at home, with reduced demand for hospital and other health and care services.

Current services are fragmented and commissioned separately from a number of different providers.

NHS North Durham Clinical Commissioning Group (CCG) and NHS Durham Dales, Easington and Sedgefield CCG intend to undertake a procurement process for the provision of adult community healthcare services.
NHS Darlington CCG working alongside partners will undertake a procurement process for the provision of adult community services.

The scope of some of these services will be locality based and others potentially be County Durham and Darlington-wide, but with some local variation. A key requirement will be the ability to partner with a wide range of organisations including general practices, voluntary and private sector providers and statutory organisations to successfully integrate care pathways between primary, secondary and community services using the Primary Care Home model or similar. Provider(s) will be expected to work across the health and social care economy to deliver high quality and value for money services ensuring a seamless patient pathway. The provider(s) will be expected to move to a model of service delivery which is outcome-based, with a performance management framework referencing KPIs and benefits taken from across the integrated health and social care system. In doing so, the provider(s) must have in place suitable arrangements with the supply chain of other providers/partners (where necessary) and agreements to work collaboratively with other organisations. There is an expectation that providers will be working to establish these relationships in anticipation of participating in the procurement process. We expect that the procurement process will have concluded by April 2018 with an anticipated contract start date of 01.10.2018. The Contracting Authorities have yet to determine the optimum procurement and contracting model to be used. For example, it may be preferable for provider(s) to hold separate contracts with each commissioning partner, to have a contract to deliver specific services across CCGs or to have a single contract across all commissioning partners. Other options are available. The Contracting Authorities are looking to engage with interested parties to explore possible strengths and weaknesses of different models through completion of a Request for Information (RFI) and an engagement event scheduled for 07.09.2017 (venue to be confirmed). An Expression of Interest (EOI) is no indication of a commitment to participate in any potential future tender process nor does it infer any preferential or special status on those organisations who do submit an EOI.
