Description
Greenwich CCG is inviting bids from Prime Contractors/Prime Providers to deliver a Community Pharmacy Medicines Optimisation Service, The Prime Provider/Contractor will recruit and manage approximately 20 providers to deliver the service from multiple sites within the Royal Borough of Greenwich ensuring that the providers are fully compliant with the NHS Pharmaceutical and Local Pharmaceutical Services regulations 2013 for the delivery of Essential Services and be registered with the General Pharmaceutical Council (GPhC).
Whilst the Commissioner will accept a phased roll out of the service, a minimum of 6 sites must be ready to deliver the service on the service commencement date of 5 September 2016 and the full compliment of sites, ( approx. 20), must be able to accept referrals from 3 October 2016
The service will, accept referrals from GPs and other professionals and carers as well as self referrals and will, maximise the effectiveness of prescribed medicines by reviewing the use of compliance aids, inhaler technique and blood glucose meters for Type 2 diabetes patients who self monitor.
The pharmacists who undertake the service should comply with his or her obligations under Schedule 4 to the Pharmaceutical Services Regulations (terms of service of NHS pharmacists) in respect of the provision of essential services and an acceptable system of clinical governance. The pharmacists should have an MUR certificate and be able to speak to patients in an area for confidential consultations at the pharmacy premises,
