Description

Chronic Obstructive Pulmonary Disease (COPD) is a chronic progressive lung condition. In 2014 there were 12,169 patients within Bradford, Airedale, Wharfedale and Craven registered as having COPD (QOF) with an overall prevalence of 2% (BDCCG 2.3%, BCCCG 1.1% and AWC 2%) higher than the national prevalence of 1.8%.

The expected prevalence in Bradford and Airedale is expected to rise to 4.5% by 2020.

The Department of Health's costing model for Pulmonary Rehabilitation (PR) services estimates that there will be 100 new case of COPD for every 100,000 population per year. This equates to 581 new diagnosis of COPD each year in B&AWC of which 28% (MRC 3-5) meet referral criteria for PR (162 patients per year)

The service will provide pulmonary rehabilitation in a variety of community settings, group work programmes will incorporate physical training, disease education, nutritional, psychological and behavioural intervention whilst promoting self-care at all times.

Objectives

• to promote and embed pulmonary rehabilitation as an essential component in the management of patients with COPD and other chronic respiratory conditions

• to improve the understanding amongst health professionals of which patients will benefit and should be referred to pulmonary rehabilitation

• to improve access to pulmonary rehabilitation for eligible patients

• to provide a cost effective, quality assured pulmonary rehabilitation programme that meets the patient's needs

• to provide proactive and flexible maintenance classes

• To ensure users of the service have a positive experience

All provisions of this service will be expected to meet and comply with NHS standards for IT Governance, Data Protection, security of general and personal information, data and records held and processed and transmitted (via and over all mediums).

The successful provider will be required to sign the NHS Standard Contract for the service and accept all terms and conditions.
