Description
HRW CCG intend to commission a Domiciliary Care Service for Hambleton and Richmondshire. The service described within this specification forms part of the integrated end of life pathway for the Hambleton and Richmondshire locality only.
A rapid relief team will support the community team to deliver end of life care to patients at home. This team will consist of Health Care Assistants (HCA's) which will be centrally co-ordinated and delivered. This service will be drawn down by the integrated locality team following the identification of a care need and ceased once the care requirements are no longer needed.
The Contract will be for a term of three years with the opportunity to extend up to 24 months subject to mutual agreement between the Provider and the Authority on the basis of two 12 month extensions (3+1+1). The contract value is stated as the maximum should contract extensions be utilised.
