Description
The service will support the mental health of individuals, families and communities by operationalising an additional support option when patients have accessed Bradford A&E and shown signs that a call to check in on the patient might be helpful. The service will test the need and outcomes of a different support mechanism and assist appropriate triage to the right support services.
