Description

The purpose of this tender is to appoint an appropriately qualified provider to develop and maintain a high quality data management system for the stop smoking service element of the Live Life Better Derbyshire (LLBD) service. The data collected will be used to:
• Follow and record the client pathway from time of referral to discharge
• Produce reports
• Submit electronic quarterly returns to NHS Digital
• Measure performance against local targets
• Provide intelligence to inform delivery of the service
• Record Nicotine Replacement Therapy (NRT) given to clients & to produce reports on NRT usage.

The contract is for 5 years, with an expected implementation period between Mid-September to November 2017 and the provision to commence on 1st December 2017.
