Description
PH0011-3 - School Nursing Service
Sandwell MBC wishes to tender for a high quality School Health Nursing Service that will improve outcomes for children, young people and families in Sandwell. The service model is based on the Department of Health's evidence-based programme of best practice; Healthy Child Programme, and will provide an invaluable opportunity from early in a child's life to identify families that are in need of additional support and children who are at risk of poor outcomes.
The aim of the School Health Nursing service is to provide a key role in the reduction of child health inequalities through the provision of health promotion and education, early identification of needs, targeted interventions and signposting to other services for school-aged children.
This will be achieved by focusing on 6 high impact areas:
1. Working closely with schools, parents and local services to build resilience and improve emotional health and wellbeing of school aged children.
2. Contributing as part of a multi-agency team to safeguarding, including keeping safe, managing risk and reducing harm, including child sexual abuse and all forms of exploitation.
3. Taking a lead role in the delivery of health education and monitoring of child health and wellbeing (including NCMP).
4. Contributing as part of a multi-agency team, to help children realise their potential, maximising learning and achievement, particularly those with multiple needs and whom are vulnerable (including children in care, young carers and young offenders)
5. Leading support for children and young people with additional health needs including education, training and support for families, carers and school staff.
6. Supporting seamless transition into school, as well as transition and preparation for adulthood.
