Description

The aim is to deliver the diagnostic skills of consultant dermatologists directly to GP practices through an N3 hosted telemedicine app. Once advice is deemed necessary by the general practitioner or GP based clinical nurse specialist, every skin related referral can be sent through this channel using practice HCA's and administration staff to upload histories and pictures of skin lesions, wounds or rashes.

The key drivers for the development of a community dermatology service are to provide a local, more accessible and cost-effective service for patients:

• Improving Outcomes for People with Skin Tumours

• Integrated Service Delivery in Dermatology

• High Quality Care for all

The telemedicine service can then offer clinical advice, (including messages, educational videos to GP's, patients and nurses, or remote real-time face to face consultation) to help with:

• Remote diagnosis and reassurance

• Remote diagnosis and advise appropriate pathway for skin cancers

• Diagnosis, investigations and treatment plan for wounds - making sure wounds are diagnosed correctly to support community care services and to provide "time to heal" date i.e. the date by which that wound should have healed

• Diagnosis and treatment plan for medical dermatoses (rashes)
