Description
Croydon Council are seeking to commission a service provider to deliver an accessible weight management service for children and young people in the borough aged 2-16 years of age. This service will deliver both a tier 1 universal and prevention service and tier 2 intervention service. Commissioners are seeking to commission an outcome focused service which will specifically ask the future provider to: • Provide an integrated approach to tackling the issue of healthy lifestyles and weight within this age bracket. • Be primarily focused on providing an evidence based and accessible weight management intervention service for children and young people aged 2-16 years who have a BMI at the 91st centile or above (Performance target will be to support a minimum of 350 children or young people and their families through the programme on an annual basis). • Make links and ensure good integration with those delivering Tier 1 services such as leisure services, parks, schools, the Healthy Schools network, school food plan initiatives and others. • Provide training to the wider workforce on how to deliver brief interventions, identify and tackle the issue of healthy weight and make appropriate referrals to services which are available in the borough • Target extra support at groups who have greater need, such as those living in deprived areas, children with disabilities, children with BAME (Black, Asian, Minority, and Ethnic) communities etc. • Develop referral networks, for example with schools, school nurses, children centres and others so that people across the borough know who and how to refer The Council requires the successful Service Provider to comply with the London Living Wage
