Journey Details: Start Date: 19/07/2018 End date: TBC From: Berwick Road, Marlow, Bucks, SL7 3AT To: Bourne End Day Centre, Wakemand Road, Wooburn Green, Bourne End, SL8 5SX Days: Thursdays Pick-up: 09:30 Return: 15:00 Client Details: Client uses a Council-approved wheelchair to mobilise. Client has to come down one step to get out of the property (and up the step to get back in) and will need assistance with this. Client is able to communicate clearly and understand instructions but suffers from short-term memory loss so questions and instructions will need to be repeated. Client's spouse will be there for the pick-up and upon returning, client will use a key to open the door. Transport Requirements: - Wheelchair Accessible Vehicle - Driver must hold a current Bucks CC ID badge, be ACTT trained, CRB/ DBS checked and English speaking - No PA Required - Please can you inform us by Monday 16th July 2018, no later than 9.30am
