Description

NHS Doncaster CCG is looking to appoint lists of 'Any Qualified Providers' in the following services:

• LOT 1 - Low Risk Glaucoma Community Monitoring Service
• LOT 2 - Intra Ocular Pressure and Visual Fields Referral Refinement Service
• LOT 3 - Direct Cataract Referral and Post-Operative Assessment Service
• LOT 4 - Children's Post Vision Screening Service
• LOT 5 - Minor Eye Care Service
