Description
Home to Settle is a service being procured to support Winter Pressures in the East Kent area.
Home to Settle - known more commonly as "Assisted Discharge" - is a service which identifies patients who are non-clinical blockers who can be supported in returning home earlier. The service also includes a front-door support which helps prevent re-admission of patients, particularly elderly people who live alone.
