Description
Solihull CCG are inviting bids for a Care Navigator Service to support and enable people with frailty to find their own solutions to problems and help them to navigate and access relevant services, providing a cost effective link between people and their GP and other health and social care professionals. The service will improve people's well-being and independence and reduce unnecessary and inappropriate use of primary and secondary care services.
The Contract period will be for 2 years with an option to extend for up to a further 2 years at the discretion of the CCG - so for clarity, the contract will start 01st April 2017 with the initial period ending 31st March 2019. There will be an option to extend at the discretion of the CCG to 31st March 2021.
The spend figures given are estimates only based on historical activity they are not to be taken as a guarantee of activity.
PLEASE NOTE THERE IS A FINANCIAL THRESHOLD - NO BIDS OVER £966,000 (Base amount £942,440 plus £23,560 CQUIN) OVER 2 YEARS (INITIAL PERIOD) WILL BE ACCEPTED - ANY BIDS EXCEEDING £966,000 (2 YEAR INITIAL PERIOD) WILL BE DISQUALIFIED. CONTRACTS ARE ANNUALLY ADJUSTED IN LINE WITH NATIONAL GUIDANCE.
