Description
CCS FRAMEWORK
Adult social care (ASC) and health services in Leeds are facing significant operational and financial challenges as a consequence of a reduction in funding allocations along-side the impacts of demographic changes within the city heightening demand for services. The Council requires a Contractor with the required expertise to work in partnership with the ASC workforce and wider health and care partners to undertake a comprehensive diagnostic assessment covering: • Prevention and early intervention: the effective use of data and analytics to improve outcomes for target cohorts (e.g. falls prevention and carer breakdown) • Working Age Adults: working across Learning Disability (LD) and mental health sectors to improve outcomes and reduce demand. • Older Adults: building on the progress made in respect of the HomeFirst service to improve outcomes for older people, specifically focussing on community demand and resulting outcomes.
