Description

The service will provide assisted discharge support from Emergency Departments and adult wards and units to eligible individuals in East Riding of Yorkshire. The service will: contribute to the prevention of social hospital admission / readmission; seek to reduce hospital admissions / readmissions through prevention and early intervention; provide a rapid return to community settings; support frequent hospital attendees to access services more appropriate to need, improving signposting to statutory and voluntary organisations; reduce preventable admissions and readmissions and enable early safe discharge. The service operates at Hull Royal Infirmary and Castle Hill Hospital.
