The supported living apartment will support a hospital discharge pathway for people who are unable to return to their own home, but who are clinically fit to be discharged from hospital.
People may be unable to return to their own home because a family member is shielding.
The occupant of the apartment will benefit from a welfare check and have access to a member of staff on site 7 days a week 9 - 4:30 with Access to out of hours on call staff guidance and telephone support 24hrs per day
