Description

The resettlement service will support people with a combination of practical and emotional support during the transition from supported housing into their next step. The service will be provided following referral from a pathway provider. The resettlement service will respond quickly and will work with the current provider to ensure that the transition is successful. This may include meeting with the resident at the current accommodation, and being flexible to meet people's needs in a personalised way.
