Description
NHS Buckinghamshire CCG is seeking market feedback from providers for a GP Surgery based in High Wycombe, Buckinghamshire with a branch surgery currently located in the village of Holmer Green, Buckinghamshire (although these branch premises will not be available from 7th September 2018).
The patient list is derived mainly from two very distinct geographies as is reflected in the location of their surgeries. It comprises a high proportion of elderly patients in and around the Holmer Green area, with young BME families being prevalent in the Wycombe area. The current list size is approx. 7,600 patients as at 1st July 2018. A provider would need to be able to offer innovative solutions for patients of Holmer Green after the 7th September as the Branch premises location is no longer available.
Services to be provided:
• Essential Services
• All Additional Services
• Core opening hours and service delivery from 8am - 6.30pm Monday to Friday (except statutory bank and public holidays)
• Improved Access (8am - 8pm Monday to Friday and weekend sessions) as part of the Locality offer (this will be funded in addition to the core contract)
The practice also registers patients from care homes in the area and the CCG estimates there are just over 700 patients >75 years of age which includes those residing in care homes.
The CCG is looking for a patient-centred approach to care delivery that is user friendly and culturally sensitive and encompasses national and local public health targets. The CCG is also looking for providers who can demonstrate innovation and a willingness to deliver a number of key design principles e.g. improving access and promoting prevention and self-care, as part of core service delivery. The provider would also be expected to collaborate within a primary care network and join the GP federation covering this area.
In order to establish the market, the CCG seeks feedback from providers to see if there is an interest and/or if any challenges are anticipated through a Survey Monkey questionnaire.
