Description
The Authority is looking to commission a development programme to capture the transformation potential of personalised care in tackling inequalities in mental health and the role of the VCSE sector within that, as evidenced by NHSE and the Race Equality Foundation report building on the report's findings to support practical application of within two NHSEI regions, confirm the need for this to be replicated across local health and care systems, with an upskilled VCSE sector acting as an essential change agent in the transformation of mental health (and allied) services premised on the principles of co-produced, personalised care.
This work will deliver the following outcomes for participants:
• Increased understanding of how to utilise targeted personalised care and support to address health inequalities within mental health services commissioned and delivered within communities via VCSE organisations.
• Enhanced ability to co-produce a personal health budget offer working with communities and groups impacted by health inequalities which addresses inequality in minoritised communities, and the impact of wider determinants of health and wellbeing.
• A commitment to a culture of continuous improvement to address health inequalities through the application of personalised care: with a particular focus on the role of personal health budgets.
• Personal and professional development and contribution to a national learning set to scale and spread personalised care in addressing inequalities through personalised care planning and support for people with mental health needs, mental illness, and their unpaid carers.
