Description

Tameside is introducing a Discharge to Assess (D2A) scheme to support the hospital discharge process helping to alleviate pressure within the acute sector by facilitating the discharge process through the provision of temporary support in a residential/nursing home setting for service users medically fit for discharge. This could be for a period of 1 to 2 days or could be up to a maximum of 6 weeks. Within this period the Council will be undertaking a Care Act assessment, along with any other assessments required to ensure that the long term care and support needs of the service user are identified/required by law, e.g. nursing assessments, mental health assessments, best interest assessments, etc. Please refer to the documents for the full specification.
